Endeavor: Online Eligibility, Claim Status and Remittance Advice Available
Suppliers in California are being asked to register to begin using a free,
Web-based tool NAS DME is offering called Endeavor. Endeavor provides suppliers
access to patient eligibility 24 hours a day/seven days a week, claim status,
and remittance advices online once they have completed the registration process.

Endeavor is in a pilot/test stage at this time. The information you will receive
is driven from national eligibility files as well as our claims processing
system. Therefore, the validity of the information returned by Endeavor matches
that of the IVR and what our Customer Service Representatives have available.

benefits Endeavor offers, it is important that you retain the contact

To register for Endeavor:

1. Click here:
2. Click on Endeavor Login.
3. Click on New User Registration.
4. Read the registration requirements and accept the terms of agreements.
5. Complete the Organization, Contact and Provider pages.
6. If registering for more than one National Provider Identifier (NPI):
- Register in Endeavor for one NPI only.
- After registration is processed and fax with User ID is received,
complete the Multiple NPI Request form on this email. This form must be
completed in order to receive access to more than one NPI.

Note: Third parties must complete additional forms after registering. An email
or fax with instructions will be sent to the user after registration.

Password Requirements
- Exactly 8 characters
- At least 1 upper case alpha (A-Z)
- At least 1 lower case alpha (a-z)
- At least 1 numeric (0-9)
- At least 1 special character (@, #, $)

As the test phase for Endeavor is completed, the product will be available as a
link our Web site, http://www.noridianmedicare.com/dme/. Registering during this
test phase assists with timely registration processing by NAS and also provides
our supplier community an opportunity to share recommendations with our team.

Multiple NPI Request Form
Are you registering for more than one NPI? After you receive your User ID via
fax, complete this form to request access to other NPIs. Fax the completed form
