Situations Requiring a Narrative Explanation In certain situations, it is necessary that suppliers provide a narrative explanation on a claim to assist in proper claim processing. When a narrative explanation is necessary, suppliers are to report the required information in loop 2400 (line note), segment NTE02 (NTE01=ADD) of the ASC X12, version 4010A1 professional electronic claim format. If the supplier has an Administrative Simplification Compliance Act (ASCA) waiver on file and submits the CMS-1500 paper claim form, suppliers may report this information in Item 19 of the claim form. To assist suppliers in identifying these situations, National Government Services has developed a list of the most common situations that require a narrative explanation on a claim.
