* Claim = equiv to a HCFA 1500 form (one patient up to 6 procedures per form), 20 claim a month minimum = $13.00 per month. Minimum is for 1-20 claims are billed through Simpleclaims. Additional support is billed at $125.00 per hour. See support policy sheet for additional information.
** Eligibility check is for one patient , one plan, one date of service, 10 eligibility check a month minimum = $5.00 per month. Minimum is for 1-10 elig checks per month.
